Includes affidavit signed by comrades Lieut. Henry Gaines and Col. Ward B. Burnett, and authorized by Notary Public Louis A. Morrell of Kings County, New York. Also included is certification of Notary Public Louis A. Morrell by Kings County Clerk George G. Herman.
